How We May Use and Disclose Your Information Without Your Written Permission
Treatment. We use and share your PHI to provide and coordinate your care. For example, we may share evaluation findings or progress updates with your physician, surgeon, or another provider involved in your care.
Payment. We use and share your PHI to obtain payment for services. For example, we may prepare a superbill you can submit to your insurer for out-of-network reimbursement, or process your payment for services.
Health care operations. We use your PHI to run our practice, such as quality review, training, scheduling, record keeping, and working with vendors that support us ("business associates") under written agreements that require them to protect your information.
Communications about your care. We may contact you about appointments, services, and follow-up by phone, text, or email. Tell us how you prefer to be contacted and we will honor reasonable requests.
People involved in your care. We may share relevant information with a family member or friend involved in your care or payment, if you agree or do not object, or if we reasonably believe it is in your best interest when you cannot decide for yourself.
Other uses and disclosures the law permits or requires without your authorization:

You may revoke an authorization at any time by telling us in writing. Revoking it will not undo anything we already did in reliance on it.
More Protective Laws

Your Rights
Access. You may inspect and obtain a copy of your records, including an electronic copy where we maintain them electronically. We will respond within 30 days (one 30-day extension is allowed). We may charge a reasonable, cost-based fee.

Accounting of disclosures. You may request a list of certain disclosures we made of your PHI during the six years before your request.
Request restrictions. You may ask us to limit how we use or share your PHI. We are not required to agree, with one exception: if you pay for a service in full out of pocket and ask us not to share information about it with your health plan, we must honor that request unless the law requires otherwise.
Confidential communications. You may ask that we contact you in a particular way or at a particular place, and we will accommodate reasonable requests.
